Liability Waiver

(Liability Waiver and Massage Intake Form must be signed before your first session begins. You will receive a request to complete them online when you book. )

• I understand that my services today are not a substitute for medical care. I understand that my therapist is not licensed diagnose or prescribe or treat physical or mental illnesses.

• I affirm that I have notified my therapist of all known medical conditions and injuries.

• I agree to inform my therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist's part should I neglect to do so.

• I understand that massage is entirely therapeutic and non-sexual in nature.

• (By signing this release,)I hearby waive and release my therapist from and and all liability, past, present and future relating to massage therapy and bodywork.

• Massage appointments must be cancelled or rescheduled (via text, phone message or email) at least 24 hours in advance in order to avoid a cancellation fee of 50% of service charge.

• No Show free is 50% of the service.

• Please note: No show policy and fee may be waived in the event of an emergency, sickness, or after evaluation of extenuating circumstances.
